How to Register by Fax or Mail

The HELIER NDT course enrollment form is provided in PDF format and may be opened in Adobe Acrobat Reader. If you do not have Acrobat Reader, it is available free here.
2. Complete the course enrollment form and...
If paying by credit card, fax the form to the HELLIER office that corresponds with the location of your chosen course.
Fax numbers are included on the enrollment form.
OR
If paying by check, mail the form along with your check to the HELLIER office that corresponds with the location of your chosen course. Mailing addresses are included on the enrollment form.
3. Upon receipt of your enrollment form we will confirm your registration.
NOTE: Space and availability of HELLIER NDT training classes are on a first come, first serve basis. Therefore, registration by mail may not guarantee your space. Registering online or by fax are the best options for securing your spot.
